Due to the high volume of patients we see, expect to schedule your eye exam appointment several weeks in advance.
Please bring a photo ID, a list of your current medications, and any insurance cards with you for your appointment.
If your insurance plan requires a referral, please request one from your PCP prior to your appointment.
